IUD (Mirena, Paragard)
IUD, or intrauterine device, is a reversible form of birth control. IUDs are T-shaped plastic devices that are progesterone or copper- releasing. Copper in the uterine cavity is detrimental to sperm mobility. Progesterone released in the uterine cavity creates changes in the lining of the uterus that impair fertilization and implantation. The copper-releasing version is effective in the uterus for 10 years. The progesterone-releasing version is effective for 5 years.
Effectiveness of IUD
Effectiveness is one of the most important concerns when deciding on which form of birth control to use. IUDs are very effective in preventing pregnancy. If they are used correctly less than 1 out of 100 women will get pregnant who use an IUD in a year. However, it is very important to remember that they do not protect against sexually transmitted diseases.
Safety and Side Effects
Twenty percent of women discontinue use of the IUD in the first year due to bleeding, cramping pain, or spontaneous loss of the IUD. Most progesterone-releasing IUDs will actually make periods lighter than usual by making the lining of the uterus extremely thin. They cannot be placed in the presence of a pelvic infection and therefore screening for sexually transmitted diseases is often performed prior to their placement. Some instances when IUDs should not be used are:
- Pelvic infections
- Sexually transmitted infection
- Cervical cancer
- Cancer in the uterus
- Pregnant
- Uterine perforation during IUD placement
- Unexplained vaginal bleeding
- Breast cancer or liver disease (for hormonal IUD use)
In some rare instances a healthcare professional might find that abnormalities in the shape of a woman’s uterus may not allow for IUD placement.
